تمایز سلول های بنیادی مزانشیمال با القای ذرات نانو تیوب تیتانیوم و اشعه فرابنفش به سلول های جنسی
مقدمه و هدف: بسیاری از درمان های ناباروری به علت فقدان گامت های جنسی بیماران کارآمد نیستند. تمایز سلول های بنیادین به سلول های زایا ی جنسی می تواند گزینه مناسبی برای تولید گامت و کاربرد کلینیکی برای درمان ناباروری باشد. مواد و روش ها: سلول های بنیادین مزانشیمال به روش فلاشینگ از استخوان ران موشهای بالغ نژاد NMRI تهیه گردید. از ذرات نانو تیوب تیتانیوم titanium nanotube particles (TNT)، پرتو UV با فرکانس (254 ± 20nm) و رتینوییک اسید برای تمایز استفاده شد. ویژگی ساختاری TNTو فیبرین با میکروسکوب FE- SEM و SEM ارزیابی شد. از مارکر های CD90,CD31 برای تایید ماهیت سلول ها و برای بیان زایایی سلولهای تمایز یافته جنسی از مارکر MVH استفاده شد. آزمون های ANOVA و Mann-Whitney برای تحلیل آماری داده ها بکارگرفته شد. نتایج: ساختار یکنواخت TNT با ابعاد 10-15 نانومتر با کمک FE- SEM مشخص گردید. تصاویر SEM پوشیده شدن TNT روی فیبرین را نشان داد. در MTT بیشترین میزان بقای سلول ها در غلظت TNT 50 µg/ml دیده شد. ماهیت سلول های بنیادین با بیان مارکر های اختصاصی با روش فلوسایتومتری مشخص گردید. رنگ آمیزی های Alizarin red رسوب کلسیوم و Oil red با تایید قطرات چربی ماهیت چند توانی سلول های بنیادی مزانشیمی را اثبات کرد. با روش ایمنوهیستوشیمی بیان مارکر MVH در سلولهای تمایز یافته در گروه های درمانی نسبت به کنترل تایید گردید. نتیجهگیری: کاربرد TNT و UV روی سلول های بنیادی مزانشیمال موجب افزایش معنی دار تمایز سلول های زایای جنسی می شود.

تاثیر چهار هفته تمرین استقامتی به همراه مکمل یاری پروبیوتیک بر بیان ژن Bax و Bcl-2 در کاردیومیوسیت رت های دیابتی
مقدمه و هدف: افزایش آپوپتوز کاردیومیوسیت ها از عوامل اصلی ایجاد کاردیومیوپاتی دیابتی است. با توجه به نقش پروبیوتیک ها و تمرینات ورزشی در بهبود عوارض ناشی از دیابت، هدف از پژوهش حاضر بررسی تاثیر هم افزای تمرین استقامتی و مکمل پروبیوتیک بر بیان ژن های Bax و Bcl-2 در کاردیومیوسیت های رت های دیابتی بود. مواد و روش ها: 30 سر رت نر نژاد ویستار با میانگین وزن 10±270 گرم، به صورت تصادفی به 5 گروه: کنترل سالم (NC)، کنترل دیابتی (DC)، مکمل پروبیوتیک دیابتی (SDC)، تمرین دیابتی (TD) و مکمل پروبیوتیک - تمرین دیابتی (STD) تقسیم شدند. گروه های تمرینی با شدت60-65 درصد vVO2peak، 5 روز در هفته به مدت 4 هفته بر روی تردمیل دویدند. همزمان گروه های مکمل، روزانه 2 گرم مکمل پروبیوتیک دریافت کردند. بیان ژن های Bax و Bcl-2 در بطن چپ رت ها به روش qReal-Time PCR، و تجزیه و تحلیل داده ها با آزمون آنالیز واریانس دوطرفه با سطح معنی داری 05/0>p انجام شد. نتایج: بیان ژنBax در همه گروه ها نسبت به گروه DC کاهش داشت (05/0<p) اما در گروه TD نسبت به STD تفاوت معنادار نبود ( 918/0=p). همچنین بیان ژن Bcl-2 در همه گروه ها نسبت به گروه DC افزایش یافت (001/0=p) اما در گروه TD نسبت به STD این افزایش معنادارتر بود (002/0=p). نتیجهگیری: به نظر می رسد تمرین استقامتی به همراه مکمل یاری پروبیوتیک با تاثیر هم افزا بر برخی عوامل موثر بر آپوپتوز کاردیومیوسیت ها می تواند در کاهش عوارض ناشی از دیابت در کاردیومیوسیت رت های دیابتی موثر باشد.

بررسی اثر تداخلی دوزهای پایین کتامین با سندروم تخمدان پلی کیستیک القاء شده با مرفین در موش بزرگ آزمایشگاهی
مقدمه و هدف: مرفین احتمالاً با تغییر سطح گلوتامات در هسته های هیپوتالاموسی باعث القایPCOS) Polycystic ovary syndrome ) میشود. در این پژوهش اثر تداخلی دوزهای پایین کتامین با PCOS ناشی از مرفین بررسی شد. مواد و روش ها: 48 سر موش ماده نژاد ویستار باکره در محدوده وزنی 220-250 گرم انتخاب و به طور تصادفی در هشت گروه دسته بندی شدند. گروه اول مرفین (5 میلی گرم بر کیلوگرم) به صورت داخل صفاقی دریافت کرد. گروههای دوم تا چهارم کتامین دریافت کردند (1، 2، 4 میلی گرم بر کیلوگرم). در گروههای 5 تا 7، کتامین (1، 2، 4 میلی گرم بر کیلوگرم) 20 دقیقه جلوتر از مرفین (5 میلی گرم بر کیلوگرم) تزریق شد. گروه کنترل فقط سالین (1 میلی لیتر بر کیلوگرم) گرفت. پس از 48 ساعت، نمونههای خون و سرم برای تعیین سطوح هورمونی تهیه گردید و تخمدان و رحم حیوانات بیومتری و به کمک هماتوکسیلین-ائوزین رنگ آمیزی شدند. تجزیه و تحلیل آماری با آنالیز واریانس (ANOVA) تحت 05/0=α انجام شد. نتایج: مرفین و کتامین تنها موجب القای PCOS شدند. با این حال، تجویز کتامین قبل از مرفین، باعث کاهش معنی دار تعداد کیست در تخمدان گردید. همچنین در گروه دریافتکننده مرفین، افزایش نسبی قطر رحم مشاهده شد که این امر به دلیل پیش تیمار کتامین متوقف شد. این اثرات با کاهش سطح FSH و تغییر مقادیر استروژن و پرولاکتین ارتباط نشان داد. نتیجهگیری: اثر تداخلی کتامین با مرفین کاملاً به تغییر در سطح هورمون گنادوتروپین بستگی دارد.

مقایسه اثربخشی شناخت درمانی مبتنی بر ذهن آگاهی و درمان مثبت نگر ایرانی-اسلامی بر فاجعه پنداری درد و شدت علائم زنان مبتلا به سندرم روده تحریک پذیر
مقدمه و هدف: سندرم رودهی تحریکپذیر شایعترین، پرهزینهترین و ناتوان کنندهترین اختلال عملکردی دستگاه گوارش است. عوامل روان شناختی تأثیر عمیقی بر شروع، طول مدت و شدت این بیماری دارد. این پژوهش با هدف مقایسه اثر بخشی شناخت درمانی مبتنی بر ذهن آگاهی و درمان مثبت نگر ایرانی– اسلامی بر فاجعه پنداری درد و شدت علائم در زنان مبتلا به سندرم روده تحریک انجام شد. مواد و روش ها: روش پژوهش حاضر، نیمه تجربی و طرح پژوهش سه گروهی دو مرحله ای (پیش آزمون و پس آزمون) بود. نمونه مورد مطالعه شامل 45 نفر از زنان مبتلا به سندرم روده تحریک پذیر در دامنه سنی 18 تا 50 سال در سال 1398 بودند که به روش نمونه گیری هدفمند مبتنی بر ملاکهای ورود و خروج نتخاب و با انتساب تصادفی در سه گروه (دو گروه آزمایش و یک گروه کنترل) جایگزین شدند. برای سنجش متغیرهای پژوهش از ابزار تشخیصی ROME III، مصاحبهی ساختار یافتهی تشخیصی برای ) DSM-V-RV SCID-I)، پرسشنامه فاجعه پنداری درد (PCS) و پرسشنامه شدت علائم سندرم روده تحریک پذیر (BSS-FS) در مراحل پیش آزمون و پس آزمون استفاده شد. گروه شناخت درمانی مبتنی بر ذهن آگاهی و درمان مثبت نگر ایرانی اسلامی به مدت 8 جلسه تحت درمان قرار گرفتند و گروه کنترل در لیست انتظار باقی ماندند. دادهها با استفاده از نرم افزار SPSS23 و تحلیل کوواریانس تحلیل شدند. نتایج: نتایج نشان داد که بین شناخت درمانی مبتنی بر ذهن آگاهی با درمان مثبت نگر ایرانی-اسلامی و گروه کنترل در پس آزمون در متغیر فاجعه پنداری درد تفاوت معنادار وجود دارد (p <0/01). و همچنین بین شناخت درمانی مبتنی بر ذهن آگاهی و درمان مثبت نگر ایرانی-اسلامی با گروه کنترل در متغیر شدت علائم تفاوت معنادار وجود دارد(p <0/01)، اما بین دو درمان با یکدیگر تفاوت معناداری به دست نیامد(p>0/05). نتیجهگیری: بر پایه یافتههای این پژوهش، به نظر میرسد که شناخت درمانی مبتنی بر ذهن آگاهی و درمان مثبت نگر ایرانی-اسلامی میتواند به عنوان درمانهای روانشناختی بر کاهش فاجعه پنداری و شدت علائم در زنان مبتلا به سندرم روده تحریک پذیر استفاده شود.

بررسی درصد و تعداد سلول های CD3- CD56+CD16+NK خون محیطی در زنان باردار دارای جنین IUGR
مقدمه و هدف: محدودیت رشد داخل رحمی (IUGR)، یکی از مشکلات بارداری است که تغییرات نامناسب عروق در این دوران، می تواند باعث بروز آن شود. با توجه به نقش سلول های NK در IUGR و مشکلات مطالعه آن ها در بافت، بررسی آن ها در خون محیطی ضروری است. هدف ما در این مطالعه بررسی درصد و تعداد سلول های NK+16CD+56CD-3 CD خون محیطی در زنان باردار دارای جنین IUGR است. مواد و روش ها: از 18 زن باردار با جنین IUGR و15 زن باردار با جنین سالم دعوت شد. تعداد و درصد سلول های NK+16CD+56CD-3 CD خون محیطی در دو گروه به روش فلوسایتومتری بررسی شد. نتایج: درصد سلولهای NK+16CD+56CD-3 CDدر گروه بیمار و سالم، به ترتیب 11/24 و 17/26 مشاهده گردید. تعداد این سلول ها در گروه بیمار 27/97 و در گروه سالم 7/129 مشاهده گردید. با آنالیز آماری من- ویتنی تفاوت معنی داری بین دو گروه مشاهده نگردید. با آزمون اسپیرمن، ارتباط معنی داری بین سن و سلول های NK در گروه بیمار مشاهده شد. نتیجهگیری: مقایسه درصد و تعداد سلولهای NK+16CD+56CD-3 CD در گروه بیمار با گروه کنترل، تفاوت معنی داری نداشت که می تواند به دلیل عدم آنالیز زیرگروه های سلول NK باشد. در مطالعات بعدی آنالیز زیرگروه های NK بررسی گردد. با توجه به ارتباط معنی دار سن با تعداد و درصد سلول های NK در گروه بیمار، به نظر می رسد افزایش سن به عنوان یک فاکتور منفی در بارداری، با کاهش این سلول ها همراه است.

تاثیر تمرینات مقاومتی کل بدن و مکمل دهی قره قات بر ترکیب بدن و پروتئین واکنشگر C در زنان چاق
مقدمه و هدف: چاقی و سطوح بالای مارکر های التهابی با افزایش خطر بیماری های مزمن همچون دیابت نوع 2 و بیماری قلبی عروقی ارتباط دارند. هدف این مطالعه بررسی تاثیر تمرینات مقاومتی کل بدن و مکمل دهی قره قات بر برخی شاخص های ترکیب بدن و سطح سرمی پروتئین واکنشگر Cدر زنان چاق بود. مواد و روش ها: در این مطالعه نیمه تجربی 52 زن غیر فعال با میانگین سن 9/4±1/39 سال و شاخص توده بدن 1/3±76/33 کیلوگرم بر متر مربع تصادفی در چهار گروه تمرین، مکمل، تمرین+مکمل و کنترل قرار گرفتند. افراد گرو های تمرین و تمرین+مکمل به مدت 8 هفته در یک برنامه تمرینات مقاومتی کل بدن شرکت کردند. افراد گرو های مکمل و تمرین+مکمل روزانه دو عدد کپسول 500 میلی گرمی مکمل قره قات مصرف کردند. قبل و بعد از 8 هفته مداخله، سطح سرمی پروتئین واکنشگر C، وزن، نمایه توده بدن، دور شکم و درصد چربی بدن ارزیابی شدند. تجزیه و تحلیل داده ها با استفاده از تحلیل کوواریانس انجام شد. نتایج: پس از مداخله، وزن، نمایه توده بدن، دور شکم و درصد چربی بدن در گرو ه های تمرین و تمرین+مکمل نسبت به گروه کنترل به طور معنی داری کاهش یافتند (05/0˂p). سطح سرمی پروتئین واکنشگر C بین گروه ها تفاوت معنی داری نداشت (05/0˃p). نتیجهگیری: به نظر می رسد تمرینات مقاومتی کل بدن به تنهایی یا همراه با مصرف مکمل قره قات می تواند موجب بهبود ترکیب بدن در زنان چاق شود، با این حال ممکن است به تغییر سطح سرمی پروتئین واکنشگر C منجر نشود.

هدف: در این پژوهش، تعیین شرایط و نحوه تأثیر عوامل ایجادکننده تقاضای القایی برای بازارهای مراقبتهای سلامت ایران مورد بررسی قرار گرفت. مواد و روش ها: برای ارزیابی وضعیت تقاضای القایی، شرایط تعادلی برای تقاضا و عرضه مراقبتهای سلامت در مجموع 31 استان کشور و در یک دوره 6 ساله (سالهای 1388-1393) مورد توجه قرار گرفته است. مدلهای ارزیابی، براساس مدل رگرسیونی دادههای پانلی در چارچوب مدل خطا- تصحیح با روش معادلات همزمان توضیح داده شده است. تخمین توابع تقاضا و تقاضای اضافی، در جامعه شهری تحت پوشش سازمان بیمه سلامت ایران، نتیجه ارزیابی برای تقاضای اضافی به صورت تقاضای القایی را نشان داده است. نتایج: یافتههای پژوهش نشان میدهد فراهمآورندگان مراقبتهای سلامت سرپایی و بستری به عنوان کارگزار تقاضاکنندگان برای این مراقبتها، شرایط تقاضای اضافی را مطابق با مدل عرضهکنندگان این مراقبتها تعیین مینمایند. نتیجهگیری: براساس مدلها و یافتههای پژوهش میتوان جهتگیریهای اساسی در سیاستگذاریها برای کنترل تقاضای القایی در بازار سلامت کشور را شناسایی نمود.

اثر تمرینات ترکیبی با رویکرد "نهایت آمادگی" و مصرف قهوه بر سطوح سرمی آنزیم های کبدی مردان میانسال مبتلا به کبد چرب غیرالکلی
مقدمه و هدف: رسوب چربی در کبد به میزان بیشتر از 5 درصد وزن کبد در غیاب مصرف قابل توجه الکل، عفونت های ویروسی و هرگونه علت شناسی بیماری کبدی را کبد چرب غیر الکلی می نامند. هدف پژوهش حاضر، بررسی اثر تمرینات ترکیبی با رویکرد "نهایت آمادگی" و مصرف قهوه، بر سطوح آنزیم های کبدی مردان میانسال مبتلا به کبد چرب غیرالکلی بود. مواد و روش ها: پژوهش حاضر نیمه تجربی و با طرح پیش آزمون و پس آزمون انجام گرفت. 44 مرد میانسال مبتلا به کبد چرب به طور هدفمند انتخاب و به صورت تصادفی به چهار گروه: تمرین ترکیبی(11نفر)، مصرف قهوه(11نفر)، تمرین ترکیبی+مصرف قهوه (11نفر) و کنترل (11نفر) تقسیم شدند. سونوگرافی و آزمایش خون جهت تعیین میزان آنزیم های کبدی قبل و بعد از اجرای پروتکل تحقیق انجام گرفت. گروه های تمرین به اجرای برنامه ترکیبی به مدت هشت هفته و به صورت سه جلسه درهفته به طور منظم پرداختند. آزمودنی های گروه مصرف قهوه و تمرین ترکیبی+مصرف قهوه یک روز در میان، به میزان 10 گرم قهوه دریافت کردند. داده ها با استفاده از آزمون کوواریانس چند متغیره( مانکووا) تحلیل شد. نتایج: سطوح سرمی آنزیم های کبدی در هر سه گروه آزمایش نسبت به گروه کنترل، کاهش معناداری داشت (05/0 >P). در گروه تمرین ترکیبی+ مصرف قهوه، کاهش آنزیم آلانین آمینو ترانسفراز (ALT) بیشتر بود و در سایر موارد تفاوت معنا داری مشاهده نشد. نتیجهگیری: مداخله تمرین ترکیبی با رویکرد نهایت آمادگی به همراه مصرف قهوه در کاهش آنزیم های کبدی موثر است و می توان برای افراد میانسال کم تحرک مبتلا به کبد چرب که در آستانه ورود به سالمندی هستند، استفاده نمود.

تاثیر 12 هفته تمرین استقامتی و مکمل ال آرژنین بر سطوح دی متیل آرژنین نامتقارن (ADMA) و نیتریک اکساید در زنان یائسه دارای پرفشار خونی
مقدمه و هدف: تمرین ورزشی و مداخلات تغذیه ای از قبیل مصرف ال آرژنین نقش موثری در کنترل پرفشار خونی دارند. بنابراین، در پژوهش حاضر تاثیر 12 هفته تمرین استقامتی و مکمل ال آرژنین بر سطوح دی متیل آرژنین نامتقارن (ADMA) و نیتریک اکساید (NO) در زنان مبتلا به پرفشار خونی مورد بررسی قرار گرفته است. مواد و روش ها: 40 زن یائسه دارای پرفشار خونی (56/2±26/55 سال، شاخص توده بدن: 18/1±61/28 kg.m2) به صورت تصادفی در چهار گروه (10 نفری) شامل گروه های دارونما، ال آرژنین، تمرین و تمرین+ال آرژنین تقسیم شدند. برنامه تمرین استقامتی به مدت 12 هفته با 75-60 درصد ضربان قلب بیشینه اجرا شد. مصرف ال آرژنین نیز روزانه 6 گرم در نظر گرفته شد. خونگیری در دو مرحله پیش آزمون و پس آزمون (48 ساعت بعد از جلسه آخر تمرین یا مصرف ال آرژنین) انجام و سطوح ADMA و NO به روش الایزا اندازه گیری شد. تجزیه و تحلیل داده ها با نرم افزار SPSS24 و آزمون آنالیز کوواریانس انجام گردید. نتایج: سطح ADMA در گروه تمرین و تمرین+ال آرژنین نسبت به گروه دارونما و ال آرژنین کاهش معناداری نشان داد (05/0>p). سطح NO در همه گروه ها در مقایسه با گروه دارونما به صورت معنادار افزایش یافت (05/0>p). با وجود کاهش فشار خون سیستولی در گروه های ال آرژنین، تمرین و تمرین+ال آرژنین در مقایسه با گروه دارونما (001/0>p)، کاهش فشار خون سیستولی در گروه تمرین+ال آرژنین نسبت به گروه تمرین (001/0>p) و ال آرژنین (05/0>p) نیز معنادار بود. نتیجهگیری: مصرف ال آرژنین اثرگذاری تمرین استقامتی در کاهش فشار خون سیستولی را افزایش می دهد که احتمالاً تاحدودی بواسطه افزایش بیشتر سطح NO اتفاق می افتد.

مدل ساختاری خودکارآمدی درد بر اساس سبک زندگی سلامت محور با میانجی گری درماندگی در مبتلایان به درد مزمن
مقدمه و هدف: درد تجربه ای فراگیر و آزارنده است. دردهای عضلانی- استخوانی شیوع بالایی در جوامع مختلف دارند. تشدید یا بهبودی درد علاوه بر جنبه های زیستی با عوامل روانشناختی نیز مرتبط است. هدف پژوهش حاضر تدوین الگویی ساختاری از روابط بین سبک زندگی سلامت محور و درماندگی با خودکار آمدی درد در بیماران مبتلا به دردهای مزمن بود. مواد و روش ها: پژوهش حاضر توصیفی، از نوع مقطعی است و برای بررسی رابطه ها از روش معادلات ساختاری استفاده شد. جامعه مورد نظر شامل بیماران مبتلا به دردهای مزمن شهر نجف آباد در تابستان 1399 بود که به تعداد 384 نفر طبق جدول مورگان و به روش در دسترس نمونه گیری آن انجام شد. داوطلبان به پرسشنامه های، سبک زندگی سلامت محور (HPLQ، 1988)، درماندگی آموخته شده (LHQ، 1988) و خودکارآمدی درد (2007،PSEQ ) پاسخ دادند. جهت تفسیر داده ها از نرم افزارهای 23 SPSS وAMOS استفاده شد. نتایج: سبک زندگی سلامت محور و خودکارآمدی درد، ارتباطی مثبت و قوی با هم داشتند. از طرفی درماندگی آموخته شده با هریک از شاخص های مذکور ارتباط منفی داشته و از نقش واسطه ای برخوردار بود (05/0>P). به طور کلی، مدل تحقیق دارای برازش مطلوبی بود و ساختار عاملی در نظر گرفته شده برای آن مطلوب بود. نتیجهگیری: بهبود سبک زندگی سلامت محور با کاهش درماندگی و بهبود خودکارآمدی درد همراه بوده و می تواند به عنوان راهکاریی روانشناختی در جهت درمان بیماران در کنار درمان های پزشکی مورد استفاده قرار گیرد.

مقدمه و هدف: GDNF سلول های عصبی را در مقابل التهاب و استرس اکسیداتیو ناشی از هایپرگلیسمی محافظت می کند. هدف پژوهش حاضر تاثیر تمرین هوازی به همراه مصرف ملاتونین بر بیان ژن GDNF و برخی شاخص های استرس اکسیداتیو در موش های صحرایی دارای درد نوروپاتی دیابت می باشد. مواد و روش ها: 40 سر موش صحرایی نر ویستار 8 هفته ای(محدوده وزنی3/11±204 گرم) به طور تصادفی در پنج گروه (8 = n) شامل: نوروپاتی دیابت(mg/kg50 استرپتوزوسین تزریق درون صفاقی)، نوروپاتی دیابت ملاتونین(mg/kg 10 ملاتونین روزانه به مدت 6 هفته)، نوروپاتی دیابت تمرین(30 دقیقه تمرین هوازی با شدت 15متر در دقیقه، 5 روز در هفته به مدت 6 هفته)، نوروپاتی دیابت ملاتونین و تمرین و کنترل سالم قرار گرفتند. پس از تایید ایجاد نوروپاتی دیابت توسط تست های رفتاری، پروتکل تمرین و مصرف مکمل اجرا گردید. میزان بیان ژن GDNF با تکنیک ریل تایم و شاخص های استرس اکسیداتیو در بافت نخاع با روش اسپکتروفتومتر اندازه گیری شد. آزمون آنالیز واریانس یک راهه و آزمون تعقیبی توکی برای تحلیل آماری استفاده گردید. نتایج: تمرین و ملاتونین موجب کاهش حساسیت سیستم عصبی به هایپرآلژزیا حرارتی و آلودینیای مکانیکی گردید. تمرین هوازی به همراه ملاتونین باعث افزایش معنی دار میزان بیان ژن GDNF و فعالیت آنزیم های SOD و CAT و کاهش غلظت MDA نسبت به گروه نوروپاتی دیابت شد(p <0.05). نتیجهگیری: تمرین هوازی همراه با ملاتونین میزان بیان ژن GDNF را افزایش و حساسیت نوسیسپتورها به عوامل دردزا را بهبود بخشید. پیشنهاد می شود از تمرین هوازی به همراه مصرف ملاتونین برای بیماران دیابتی به منظور کاهش درد نوروپاتیک استفاده شود
