اثربخشی مداخله جیره عاطفی بر رابطه والد-کودک و استرس والدینی مادران خردسالان با علائم شبه اُتیسم در معرض دایهگری دیجیتالی
مقدمه و هدف: در سالهای اخیر استفاده افراطی از وسایل دیجیتال در خردسالان افزایش چشمگیری داشته است و این استفاده افراطی پیامدهای نامناسب جسمانی، شناختی، هیجانی، اجتماعی و روانی از جمله تضعیف رابطه والد-کودک را به دنبال داشته است. مطالعه حاضر با هدف بررسی اثربخشی مداخله جیره عاطفی بر غنیسازی رابطه والد-کودک و کاهش استرس والدینی در مادران خردسالان در معرض دایهگری دیجیتالی انجام شد.
مواد و روشها: طرح پژوهش شبه تجربی از نوع پیشآزمون-پسآزمون-پیگیری بدون گروه کنترل بود. 12 نفر از خردسالان در معرض دایه گری دیجیتالی با علائم اًتیسم که به مرکز اًتیسم آوا مراجعه کردند بهصورت در دسترس به عنوان گروه نمونه انتخاب شدند و به مدت 2 ماه مداخله جیره عاطفی دریافت کردند. برای ارزیابی اثربخشی مداخله از فرم کوتاه مقیاس رابطه والد-کودک (1994) و فرم کوتاه مقیاس استرس والدینی (1990) استفاده شد. برای تجزیه و تحلیل دادهها از نرمافزار SPSS25 استفاده شد.
نتایج: نتایج آزمون تحلیل واریانس اندازهگیری مکرر نشان داد که شرکت در مداخله جیره عاطفی سبب کاهش میزان مواجهه خردسالان با وسایل دیجیتال، افزایش نزدیکی و کاهش تعارض در رابطه بین والد-کودک، کاهش استرس و آشفتگی والدین و نیز بهبود تعاملات ناکارآمد بین والدین و کودک شده است. اندازه اثر مداخله بر کاهش میزان استفاده از وسایل دیجیتال بزرگ و بر مؤلفههای استرس والدینی و نزدیکی-تعارض رابطه بین والد-کودک متوسط بود و اثربخشی پس از گذشت دو ماه از اتمام مداخله پایدار بود.
نتیجه گیری: بر اساس یافتهها، آموزش به والدین برای غنیسازی عاطفی رابطه با کودک در معرض دایه گری دیجیتالی با کاهش میزان مواجهه با وسایل دیجیتال، بهبود رابطه والد-کودک و کاهش استرس والدینی همراه بود؛ بنابراین از این روش میتوان برای غنیسازی زندگی خردسالانی که در معرض دایهگری دیجیتالی بودهاند، استفاده کرد.

بررسی اثرات آموزش تدابیر حفظ سلامت بر پایه طب ایرانی (حفظ الصحه) بر هزینههای درمان بیمهشدگان سازمان بیمه سلامت ایران
مقدمه و هدف: تغیر سبک زندگی انسانها باعث کاهش سلامت جسم و روان آنها و به تناسب آن افزایش بار مراجعات و هزینههای سنگین درمان و ایجاد مشکل برای دولتها برای تأمین هزینههای درمان شهروندان خود شده است.
لذا این مطالعه با هدف بررسی آموزش حفظ الصحه بر هزینههای درمان بیمهشدگان بیمه سلامت که هزینه بالا (صدک ۵۰ تا ۷۰) داشتند، انجام پذیرفته است.
مواد و روشها: این مطالعه تجربی دو گروهی با هدف بررسی آموزش حفظ الصحه بر هزینههای درمان بیمهشدگان بیمه سلامت در شهر تهران (ایران) طی سال ۱۳۹۶ انجام پذیرفته است. در این مطالعه گروه مداخله بصورت حضوری و مجازی مورد آموزش حفظ الصحه بر پایه طب سنتی ایرانی مبتنی بر مدل اعتقاد بهداشتی قرار گرفتند. گروه کنترل نیز آموزش خاصی را طی زمان مطالعه دریافت ننمودند.
نتایج: این مطالعه بر روی ۱۰۱ نفر از بیمهشدگان سازمان بیمه سلامت (۵۰ نفر مرد و ۵۱ زن) در دو گروه ۵۰ و ۵۱ نفری انجام گردید. دو گروه از نظر ویژگیهای دموگرافیک با یکدیگر یکسان بودند. نتایج نشان داد که گروه مداخله در هزینههای آزمایشگاه (72/26-%،38/2-% 001/0 P<)، رادیولوژی (92/27-%.02/5-% 001/0 P<)، داروخانه (64/23-%. 88/3% 001/0 P<)، ویزیت (52/30-%.16/16-% 001/0 P<) و همچنین هزینه کل (52/36-%. 16/1-% 001/0 P<) نسبت به گروه کنترل دارای کاهش معنیدار بوده است.
نتیجهگیری: با توجه به نتایج این مطالعه میتوان آموزش حفظ الصحه را به عنوان راهکار مناسب در کنترل هزینههای درمان توصیه نمود.

اثر تمرینات بینایی و بینایی- حرکتی منتخب بر مؤلفههای ادراک بینایی وابسته به حرکت در کودکان دارای تنبلی چشم
مقدمه و هدف: مطابق با "فرضیهی حرکت"، مسئلهی مهم در رشد و تعدیل مؤلفههای ادراک بینایی، حرکات فعالانه و یا غیر فعالانه فرد نیست، بلکه توجه فرد به اشیاء و محرکهای مهم در اطراف خود است. بر این اساس هدف از پژوهش حاضر، بررسی "فرضیه حرکت" در ارتباط با مؤلفههای ادراک بینایی وابسته به حرکت در کودکان دارای تنبلی چشم بود.
مواد و روشها: روش تحقیق در این پژوهش نیمه تجربی و در آن از طرح پیشآزمون- پسآزمون استفاده شده است. نمونههای این پژوهش شامل 30 نفر از کودکان 5 تا 8 سال دارای تنبلی چشم بودند که به صورت تصادفی از میان تمام کودکان مبتلا به تنبلی چشم شهرستان سبزوار در سال 1394 انتخاب شدند. سپس آزمودنیها بر اساس پیشآزمونها، به صورت تصادفی در سه گروه مختلف قرار گرفتند. گروه آزمایشی یک (تمرینات بینایی) و گروه آزمایشی دو (تمرینات بینایی – حرکتی) تمرینات خود را به مدت 4 هفته، 3 جلسه در هفته و به مدت 30 دقیقه در هر روز انجام دادند. گروه کنترل نیز در این مدت به فعالیتهای رومزه خود مشغول بود. در پایان نیز کلیه آزمودنیها در پسآزمون شرکت کردند.
نتایج: نتایج آزمون تحلیل کوواریانس و آزمون تعقیبی LSD نشان داد که بین گروههای آزمایشی و کنترل در مؤلفههای ادراک عمق، تیزبینی پویا و تعقیب بینایی تفاوت معنیدار آماری وجود دارد، ولیکن تفاوتی بین گروههای آزمایشی یافت نشد.
نتیجهگیری: بهطورکلی با توجه به نتایج تحقیق حاضر میتوان نتیجه گرفت که تمرینات بینایی از قبل طراحی شده (فعالانه، غیر فعالانه) موجب بهبود مؤلفههای ادراک عمق، تیزبینی پویا و تعقیب بینایی میگردد.

بررسی ارتباط شیردهی انحصاری و سواد سلامت در زنان نخست زا
مقدمه و هدف: سازمان بهداشت جهانی تغذیه انحصاری با شیر مادر را در طی شش ماه اول زندگی کودک مورد تائید قرار داده است. هدف مطالعه حاضر تعیین ارتباط شیردهی انحصاری با سواد سلامت و عوامل مؤثر دیگر در زنان نخست زا است.
مواد و روش ها: این بررسی یک مطالعه تحلیلی است. جامعه مورد پژوهش شامل زنان نخستزا واقع در سنین باروری مراجعهکننده به مراکز بهداشتی درمانی شهر تهران است که دارای کودک با سن دو سال تمام تا پنج سال بودند. اطلاعات به وسیله پرسشنامه و مصاحبه با مادران جمعآوریشده و با استفاده از مدل رگرسیون لجستیک تحلیل شد.
نتایج: شیردهی انحصاری در 5/50 درصد از مادران گزارش گردید. از نظر آماری رابطه بین شیردهی انحصاری و متغیرهای سطح سواد سلامت در سطح کافی(001/0>P < /span>)، سن مادر (001/0>P < /span>)، اولین زمان شیردهی برای سطح بیشتر از یک ساعت و سطح بعد از یک ساعت (001/0>P < /span>)، روش زایمان (05/0 >P < /span>) و شغل (05/0>P < /span>) معنیدار شد. مناسب بودن مدل با آزمون هاسمر-لمشو تائید شد.
نتیجهگیری: سطح سواد سلامت و متغیرهای زمان اولین شیردهی، سن مادر، روش زایمان و شغل بر شیردهی انحصاری تأثیرگذار بودند لذا با توجه به عوامل فوق به ویژه افزایش سطح سواد سلامت مادران از طریق برگزاری کارگاههای آموزشی میتواند مؤثر باشد.

اثربخشی افزایش خودکارآمدی زایمان به شیوه واقعیت درمانی بر انتخاب روش زایمان واژینال در زنان باردار
مقدمه و هدف: یکی از مهمترین عوامل تأثیرگذار بر انتخاب روش زایمان طبیعی در زنان باردار خودکارآمدی زایمان است. هدف پژوهش حاضر بررسی اثربخشی افزایش خودکارآمدی زایمان به شیوه واقعیت درمانی بر انتخاب روش زایمان واژینال در زنان باردار نخست زا بود.
مواد و روشها: روش پژوهش نیمه آزمایشی با طرح پیشآزمون – پسآزمون با گروه کنترل است. جمعیت تحت مطالعه شامل 39 زن باردار ساکن شهرستان نوشهر بودند که سن حاملگی 4 هفته و بالاتر داشتند، روش سزارین انتخابی را جهت زایمان برگزیده و برای مراقبت مامایی طی دیماه تا اسفندماه 96 به مراکز بهداشت شهری و روستایی مراجعه کردند. نمونه به شیوه غیر تصادفی هدفمند انتخاب و در دو گروه مداخله (20) و کنترل (19) نفر تصادفی گمارده شدند. واقعیت درمانی در 8 جلسه 5/1 ساعته دو بار در هفته برای گروه مداخله اجرا شد. از پرسشنامه خودکارآمدی زایمان لویی (CBSEL) برای جمعآوری دادهها و برای تحلیل دادهها از تحلیل کوواریانس استفاده شد.
نتایج: بعد از واقعیت درمانی، خودکارآمدی زایمان در افراد گروه مداخله در مقایسه با گروه کنترل افزایش معنیداری نشان داد (01/0>P < /span>). نمره خودکارآمدی زایمان زنانی که پس از واقعیت درمانی تصمیم به انجام زایمان واژینال گرفتند بهطور معناداری بیشتر از افرادی بودند که همچنان انجام زایمان به روش سزارین را برای ختم حاملگی در نظر داشتند (05/0>p < /span>).
نتیجهگیری: واقعیت درمانی را میتوان برای افزایش خودکارآمدی زایمان به کاربرد که خود میتواند بر انتخاب شیوه زایمان واژینال تأثیر مثبت داشته باشد.
