This study was undertaken to determine the AmpC β -lactamase production in pathogens isolated from hospitalized patients at selected university hospitals of Tehran.   Materials & Methods: Non-repeat clinical isolates (154) from clinical specimens of selected hospital were taken. Disk agar diffusion and minimum inhibitory concentration (MIC) with β -lactam antibiotic included Ceftazidim, Ceftriaxon and Cefepim and Clavulanic acid as a β -lactamase inhibitor for detection of extended spectrum β -lactamase (ESBLs) producing organisms w ere done. In addition, the susceptibility of these organisms to Cefoxitin was examined. All isolates were examined by Multiplex PCR method for finding the different kind of gene family encoded AmpC β -lactamases  Results: Isolates had divided in three group included of P < sup>+, P < sup>+C+, P < sup>+C- based on ESBLs phenotypic test criteria with 57.15% (n=88), 53.9% (n=83) and 3.25% (n=5) respectively. Isolates that their result became P < sup>+C+ and P < sup>+C- were selected and the Multiplex PCR for them were done. The results of molecular testing showed that 5.7% of isolates, in the other hand 5 strains of E. coli were positive for AmpC β -lactamase.   Conclusion: This is the first report of AmpC β -lactamase producing bacteria from Iran. If the type of β -lactamase produced by the pathogen could be detected along with antibiogram before administrating the β -lactamase drug to the patient therapeutic failure might be avoided. Because of significance of these organisms in produce nosocomial infection and in order to prevent them from spread, we need to more study for showing the actual prevalence of them.}, keywords = {β- lactamase,AmpC,Extended spectrum,Escherichia coli}, title_fa = {تعیین الگوی مقاومت ایزوله‌های بالینی اشریشیاکلی مولد بتالاکتامازهای وسیع‌الطیف نوع AmpC براساس خصوصیات فنوتیپی و ژنوتیپی}, abstract_fa = {  مقدمه و هدف: استفاده گسترده از آنتی‌بیوتیک‌های بتالاکتام موجب توسعه مقاومت به این گروه از آنتی‌بیوتیک‌ها در باکتری‌های بیماریزا از طریق تولید آنزیم بتالاکتاماز می‌شود. این مطالعه به جهت تعیین تولید آنزیم بتالاکتاماز از نوع AmpC در باکتری اشریشیاکلی جداشده از سه بیمارستان منتخب شهر تهران انجام شده است.   مواد و روش‌ها: تعداد 154 ایزوله بالینی اشریشیاکلی غیرتکراری از سه بیمارستان منتخب شهر تهران جمع‌آوری و از نظر تولید آنزیم بتالاکتاماز وسیع‌الطیف ( ESBLs ) با استفاده از آنتی‌بیوتیک‌های سفتازیدیم، سفتریاکسون و سفپیم و همچنین مهارکننده بتالاکتاماز بنام اسیدکلاولانیک به روش دیسک دیفیوژن و میزان MIC این آنتی‌بیوتیک‌ها به روش رقت در آگار مورد بررسی قرار گرفت. همچنین حساسیت این ایزوله‌ها به آنتی‌بیوتیک سفوکسیتین مورد بررسی قرار گرفت. در پایان ایزوله‌های کاندید داشتن این آنزیم به روش Multiplex PCR تحت آزمایش قرار گرفتند.   نتایج: پس از انجام آزمایش‌ها، ایزوله‌ها از نظر فنوتیپ مقاومت به سفالوسپورین‌های نسل سوم به سه گروه P + با 15/57 درصد (88= n) ، P+C + با 9/53 درصد (83= n )، و P+C - با 25/3 درصد (5 = n ) تقسیم شدند از 88 ایزولۀ بالینی ( P+C - و P+C +) که توسط روش Multiplex PCR مورد آزمایش قرار گرفتند، 7/5 درصد ایزوله‌ها (5= n ) واجد ژن‌های بتالاکتاماز نوع AmpC بودند.   نتیجه‌گیری: این اولین گزارش از وجود بتالاکتامازهای نوع AmpC از دسته EBC-M ، DHA-M و CIT-M در ایران بوده و به دلیل اهمیت ارگانیسم‌های تولیدکننده این آنزیم‌ها در ایجاد عفونت‌های بیمارستانی و برای جلوگیری از گسترش آن‌ها توصیه می‌شود تحقیقات وسیع‌تری از نظر بررسی شیوع واقعی این آنزیم در ایران انجام شود.}, keywords_fa = {بتالاکتاماز,وسیع‌الطیف,AmpC,اشریشیاکلی}, url = {https://daneshvarmed.shahed.ac.ir/article_1356.html}, eprint = {https://daneshvarmed.shahed.ac.ir/article_1356_73fa47d7912333d8e66543a65bd78620.pdf} }
